Dentistry
Dentistry involves prevention, detection, management and treatment of wide variety of conditions, disorders and diseases affecting teeth, gums and maxillofacial (associated with the jaw and face) region of the body and maintenance of oral and their maintenance in individuals. It is based on sound scientific and technical principles with the clinical aspects of dentistry underpinned by knowledge and understanding of the biological and clinical medical sciences.
Types of Dentistry
There are many different types of dentistry:-
- Restorative Dentistry : Restorative dentistry, as the name suggests, involves restoring damage to teeth brought about by decay, trauma, various systemic illnesses, inherited defects, etc. There are many types of dental restorations, but broadly speaking, they may be classified into two main categories: direct and indirect. The difference between the two is that while direct restorations are inserted into or applied to the tooth in a soft state directly in the patient's oral cavity, indirect restorations are fabricated on a model of the teeth outside of the patient's mouth. Restoration or Cosmetic dentists most commonly use 3 methods; Dental Crowns, Dental Bridges and Dental Implants. Dental bonding, dentures and porcelain dental veneers too fall under Dental Restoration procedures. Depending on the status of your teeth and the gum your dentists would decide what type of procedure is best to restore your teeth.
Endodontics : Endodontics concerns itself with treating diseases of the dental pulp. The dental pulp is the name given to the soft tissue that runs through the canal or canals in the middle of the root or roots of a tooth. The pulp consists mainly of nerves and blood vessels and its function is to provide nutrition for a developing tooth and to provide sensory feedback for an erupted tooth. This include Root Canal Treatment, the removal of the nerve of the tooth, and the sealing of the tooth to prevent further infections with a material commonly used, known as "gutta percha".
Orthodontics : It is a type of dental treatment that aims to improve the appearance, position and function of crooked or abnormally arranged teeth. Orthodontics uses mechanical devices, such as a brace, over a certain period of time (usually 18 months to two years) to correct the position of the teeth. This is the specialty which encompasses moving the teeth around the mouth using various appliances such as braces and retainers in order to improve the function of the teeth, or the aesthetics of the mouth. It also includes Acid etch, banding, debanding, bonding, cephalometric X- rays, debonding, extraoral photographs, Impressions, interceptive orthodontic treatment, ligation,ligating, panoramic Xray,etc.
- Maxillo-Facial Surgery : This encompasses any oral surgery, for example removing the remnants of broken teeth from the jawbone, or indeed teeth which are erupted, buried or impacted. Many referrals are made to maxillo-facial surgeons for removal of wisdom teeth which are impacted close to the lingual nerve, due to the danger of damaging the nerve upon extraction of the wisdom teeth. Maxillo-Facial surgeons (often called oral surgeons) also specialise in apicectomy, which is simply the removal of the apex of the tooth. The apex is the very tip of the bottom of the tooth which is embedded into the jawbone.
Implantology : A dental implant is essentially a substitute for a natural root and commonly it is screw or cylinder shaped. Each implant is placed into a socket carefully drilled at the precise location of the intended tooth. If an implant has a screw-thread on its outer surface it can be screwed into position and if it does not, it is usually tapped into place. The main aim during installation of any implant is to achieve immediate close contact with the surrounding bone. This creates an initial stability, which over time is steadily enhanced by further growth of bone into microscopic roughnesses on the implant surface. In order to support replacement teeth, dental implants normally have some form of internal screw thread or post space that allows a variety of components to be fitted. Once fitted, these components provide the foundation for long-term support of crowns, bridges or dentures.
- Periodontics : It deals with the diagnosis and treatment of gum disease. Periodontal disease (from the Latin Perio, meaning around and dontal meaning teeth) is an infection which commonly starts as gingivitis, a condition commonly making the gums inflamed and sometimes bleed when brushing or flossing. If not brought under control by routine and adequate oral hygiene, then the condition can deteriorate further into periodontitis, a condition which can alter the stability of teeth, by eating away at the surrounding jawbone, making the gums recede (hence the saying to becoming "long in the tooth"), teeth then become loose and ultimately can fall out due to having no supporting bone, if not treated properly.
- Prosthetics : Dental prosthetics is a specialist area of medicine which is concerned with the recreation of the dentition when there are missing or badly damaged teeth. Prosthetics doesn’t just mean aesthetics. A functional reconstruction is also necessary – that means it must also be capable of making sounds (phonation), chewing food (mastication) and protecting the jaw joint from the grinding of the teeth. It is the art of replacing missing teeth with artificial ones. This would include for example dentures.
- The Temporomandibular Joint (TMJ) : The temporal mandibular joint (TMJ) is the synovial joint that connects the jaw to the skull. These two joints are located just in front of each ear. This complicated joint along with its attached muscles, allows movements needed for speaking, chewing, and making facial expressions.This can sometimes “click”. There are specialist dentists who concentrate upon diagnosis and treatment of TMJ problems.

Examination and Diagnosis
- In addition to direct observation, use of dental x-rays which allow examination of areas that does not have direct visual access to, such as areas between back teeth or below the gums.
- Occasionally, laser device, called a Kavo DiagnoDent, is used to diagnose to see decay such as in the depth of deep grooves or fissures on the chewing surfaces of back teeth. Laser identifies decay on the basis that the wavelength of light reflected by a decayed tooth structure is different to that reflected by healthy tooth structure.
- For cosmetic consultations a number of very specific aspects are considered including such technical parameters as the size of teeth, the golden proportion, smile line, angulation and shape of the teeth, the incisal outline, gingival symmetry, aesthetic line, labial contour, teeth shade and many others.
- Study models are used which require to take impressions of the teeth and clinical photographs may also be taken.
- For implant treatment some additional factors are considered besides general and oral health, such as available spaces, the quantity and quality of bone, as well as aesthetic considerations. Study models of your teeth are also needed, as well as panoramic and intra oral radiographs of the teeth and jaws and sometimes a CT scan may be required.
Sedation and Anesthesia
Oral sedation before treatment, inhalational or intravenous sedation during treatment, or a general anaesthetic may be given according to the treatment and surgery requirement.
- Oral Sedation may prescribe before treatment to make treatment more comfortable. Additional sedation may be given at the time of the dental treatment to supplement the oral sedation.

General anaesthesia may be recommended for children who are extremely frightened, anxious or uncooperative.
