The bone of the jaw accepts and osseointegrates with the titanium post. The osseointegration is the component of this implant procedure that makes it resemble the look and feel of a natural tooth.


What is the Composition of Dental Implants?

A typical implant consists of a titanium screw (resembling a tooth root) with a roughened or smooth surface. The majority of dental implants are made out of commercially pure titanium, which is available in 4 grades depending upon the amount of carbon and iron contained. More recently grade 5 titanium has increased in use. Grade 5 titanium, Titanium 6AL-4V, (signifying the Titanium alloy containing 6% Aluminum and 4% Vanadium alloy) is believed to offer similar osseointegration levels as commercially pure titanium. Ti-6Al-4V alloy offers better tensile strength and fracture resistance. Today most implants are still made out of commercially pure titanium (grades 1 to 4) but some implant systems (Endopore and NanoTite) are fabricated out of the Ti-6Al-4V alloy.   • Root-Form Titanium Implant: Of all the types of dental implants, the root-form titanium dental implant is the most common. This option is also known as an endosseous or endosteal implant, a name that stems from the fact that these tooth implants are placed directly in the bone. Root-form implants are made of the reliably strong and corrosion-resistant metal titanium, making these dental implants very similar in appearance to screws, nails, or cones. Your dentist decides which type of implant to use based on the quality of the bone in your jaw and the type of crown, bridge or denture that will be placed on the implant. Root-form implants can be inserted in a two-stage process the traditional way of placing them — or in a single-stage procedure. In the two-stage procedure, the implant is "buried" under the gum tissue for three to four months and then exposed during a second surgical procedure. In a single-stage procedure, the implant is placed in the bone and remains exposed in the mouth.

A blade implant is placed in the jaw so that the prong(s) stick out into the mouth where they will support crowns or bridges. These are one of the most popular types of dental implants and have been used by dental professionals for a number of years. An endosseous implant, Blade- Form implants are flat long pieces of metals with 1 or 2 metal prongs that are implanted in your jaw bone. However, these types of implants are very versatile. They are available in different shapes and sizes and many manufacturers even willing to design a unique one for you, matching each of your specifications. Furthermore, these implants can even be shaped and bent to fit disparity in the anatomy.

  • Sub-periosteal Implant: The sub-periosteal implant is commonly used in patients who do not have enough bone in the jaw to support a root-form implant. Sub-periosteal tooth implants are placed over the jaw, rather than in it. A Subperiosteal Implant is used to secure dentures and when the bone has atrophied (receded) and jaw structure is limited. The lightweight, individually designed, metal framework fits over the remaining bone, providing the equivalent of multiple tooth roots. It may be used in a limited area or, if all the teeth are missing in the entire mouth. The amount and location of available bone determine the kind of implant that is best to use. Natural tissue membrane or bone will grow back around either implant making it even more secure.

  • Plate-Form Implant: This category of dental implant consists of a rectangle of metal with either one or two metal prongs on one side. They are placed vertically in the jaw so that the prongs stick up into the mouth and provide a place for the artificial tooth to be placed. This takes the form of a narrow metal plate which is inserted into the jawbone. Your dentist will expose the area of your jawbone for treatment before preparing the bone to receive the implant. He or she will make a series of incisions in the gums before inserting the implants. The number of incisions made will depend on the number of implants. The implants are inserted and the incisions closed with tiny stitches. The dentist will advise you to wait for a set period of time to allow osseointegration to take place. However, in some cases you can have restorations directly after the implant procedure. This is known as ‘immediate loading’.

  • Transosseous Implant: Transosseous implants originally were designed to be used in people who had very little bone in their lower jaws and who had no bottom teeth. However, they are rarely used today because placing them requires extensive surgery, general anesthesia and hospitalization. Also, their use is limited to the lower jaw. Placing transosseous implants involves inserting two metal rods from below the chin, through the chin bone, until they are exposed inside the mouth. The rods that can be seen inside the mouth are used to attach a denture.   • Two stage Dental Implants: Its procedure begins with the surgery. In this surgical process the dental implant is fixed into the jaw bone. The implant is placed in the jaw bone and is screwed in all the way so that the top surface of the “screw” (implant) is at the same level of the bone. The soft tissue is stitched up and a few months later the tissue has healed. There is no visible evidence of the implant in the bone as it is under the soft tissue and not visible to the eye. During the next appointment, the dentist will perform a minor surgery where he will expose the top surface of the dental implant, connect a dental implant abutment and start the process of the dental implant restoration. At the end of this appointment the patient will go home with the abutment in his mouth attached to the implant. Additional healing time is required, yet it is a short period and there is very little pain involved.

  • Single stage Dental Implants: is designed in such a way that it enables avoiding the second surgery. During the single stage dental implant surgery, the implant is placed in the jaw bone in such a way that the top of the implant is higher than the surface of the bone and is actually at the height of the soft tissue (this is a longer implant). When the soft tissue is stitched up at the end of the dental implant surgery the dental implant head is exposed and visible to the eye. After the healing time has passed there is no need for another surgery for exposing the top of the head of the implant because it is exposed to begin with and all the dentist needs to do is connect the dental implant abutment and start the restoration process. Both dental implant types have the same success rate of 98% and it is usually up to the dentist to decide which type of dental implants to use. There is no big difference, essentially it’s just what the dentist is used to and feels comfortable working with.

How to Prepare for Dental Implant?

Dental implant surgery is an involved process and requires several visits with specialists. Follow these steps to prepare for dental implant surgery:

  • Have a thorough evaluation. You need to get a complete dental exam with X-rays and models of your mouth.

  • Tell your dentist about any existing medical conditions. It's important he know if you have any orthopedic implants, heart or vascular problems. You may need to take antibiotics before getting the dental implants.

  • Run through your dental implant procedure plan with your dentist. Make sure you know exactly what to expect during all your surgeries.

  • Quit smoking and excessive drinking. Both are detrimental to your health and can actually lead to the dental implant procedure failing.

  • Avoid eating or drinking for at least 6 hours before your dental implant appointment.

  • Wear comfortable, loose-fitting clothing on the day of your surgery. You will be sedated, so it's important to feel comfortable.

  • Ask a friend or family member to drive you to the dental office. You will not be allowed to drive for 12 to 24 hours following the dental implant surgery.

What is the Post Operative Care following a Dental Implant?

A part of making the dental implants surgery successful is to follow your doctor’s orders during the post-operative care.  After your dental implants have been inserted you will be given antibiotics as well as antibiotic oral rinse – you need to follow all the specific instructions on how to use both the anesthetic and the oral rinse.

  • Following surgery, you will be asked to bite firmly but gently on a gauze pad to stop the bleeding.  Your doctor will give you a package of gauze to take home with you and place over the surgical site.   You’ll have to make sure to change the pads and use them until the bleeding stops completely.  

  • If you have some discomfort after the surgery you will be given a painkiller.  You can also decrease discomfort and swelling by applying an ice pack for about twenty minutes for the first six hours following surgery. You can continue applying an ice pack for as long as two days following surgery.

  • In order to minimize swelling and bleeding you may want to keep your head elevated for the first twenty-four to forty-eight hours after the surgery.  You have to make sure you relax as much as possible and avoid all strenuous activities for as long as three days following the dental implants surgery.

  • You’ll have to drink at least eight glasses of water or fruit juice every day and try to limit your diet to soft foods such as yogurt or soup during the first week of surgery.

  • You will be asked to rinse your mouth with warm salt water solution – one teaspoon of salt in one cup of water – about three to four times a day to further disinfect the area.  You have to make sure you spit carefully to avoid any complications.

  • Once your jaw bone grows around the titanium posts, you may experience a little discomfort around the area that can last for several weeks. An over-the-counter pain reliever, such as ibuprofen or acetaminophen, should make the pain go away.
